Phase 1: Protection & Isometric Loading

Approximate Timeline: 0-3 Weeks

Goals:

  • Reduce pain

  • Normalize walking

  • Maintain strength

  • Introduce safe loading

During this phase, the goal is not to completely rest the foot. Instead, we introduce controlled loading that does not significantly increase symptoms.

Exercises:

  • Toe and ankle mobility

  • Short foot exercises

  • Isometric calf holds (straight knee and bent knee)

  • Cycling

  • Swimming

  • Rowing

Avoid:

  • Running

  • Jump rope

  • Plyometrics

  • High-volume impact activities

Phase 2: Heavy Slow Resistance Training

Approximate Timeline: Weeks 3-12

Once symptoms are improving, the plantar fascia and calf complex need increased strength capacity.

Goals:

  • Improve tissue tolerance

  • Increase force production

  • Restore lower extremity strength

Exercises:

  • Heel raises with toe extension

  • Heavy double-leg calf raises

  • Bent knee calf raises

  • Squats

  • Deadlifts

  • Split squats

  • Step-ups

The focus is controlled, heavy loading.

A good target:

  • 6-8 repetitions

  • Moderate-heavy resistance

  • Approximately RPE 7-8 effort

Progress when:
- Walking is pain-free
- Morning symptoms are improving
- Daily activities no longer create a flare-up

Phase 3: Single-Leg Strength & Capacity

Approximate Timeline: Weeks 4-12

Running is a single-leg sport. Your rehab needs to eventually match those demands.

Goals:

  • Restore unilateral strength

  • Improve foot control

  • Prepare for impact

Exercises:

  • Single-leg calf raises

  • Walking lunges

  • Single-leg RDLs

  • Weighted step-ups

  • Single-leg balance variations

Milestones before progressing:

  • 20+ controlled single-leg calf raises

  • 45-60 minutes of walking without increased symptoms

  • Minimal morning stiffness

Phase 4: Elastic Loading & Plyometrics

Approximate Timeline: Weeks 6-12

The plantar fascia must be able to tolerate the elastic demands of running.

Running requires:

  • Stiffness

  • Spring

  • Force absorption

  • Energy return

Exercises:

  • Pogos

  • Jump rope

  • Lateral hops

  • Single-leg hops

  • Bounding

  • Depth jumps

  • Box jumps

Progress from:

Low Impact:

  • Double-leg pogos

  • Small hops

  • Jump rope

Moderate Impact:

  • Single-leg pogos

  • Lateral hops

  • Jump squats

High Impact:

  • Bounding

  • Depth jumps

  • Running-specific plyometrics

Progress when:
- No increase in next-day symptoms
- Morning pain remains ≤2/10
- Jumping feels symmetrical

Phase 5: Return to Running After Plantar Fasciitis

Approximate Timeline: 8-16+ Weeks

Returning to running should be based on tissue readiness, not just time.

Before returning to running, aim for:

  • Pain during activity ≤2/10

  • No limp

  • Morning pain ≤2/10

  • No symptom increase after plyometrics

  • 2 minutes of controlled single-leg hops

  • Walking and stairs are symptom-free

How Long Does Plantar Fasciitis Take to Heal?

Recovery depends on:

  • How long symptoms have been present

  • Severity of tissue irritation

  • Training demands

  • Previous injury history

  • Consistency with rehabilitation

For an acute case of plantar fasciitis (symptoms present for days to a few weeks), recovery is typically faster because the tissue has not yet developed long-standing sensitivity or reduced capacity. A realistic timeline for runners is 6-12 weeks.

For a chronic plantar fascia injury lasting 6-12 months, expect:

  • 8-12 weeks of progressive loading before meaningful improvements

  • 3-6+ months before the tissue is fully resilient for high-volume running, hills, trails, and hybrid training demands

A confirmed partial plantar fascia tear often falls toward the longer end of this timeline.

The goal is not just to feel better.

The goal is to build a foot and lower leg system that can tolerate your goals.
